Thyroidectomy (Thyroid Surgery)

Safe, Precise Surgery for Thyroid Disorders

What is the Thyroid Gland?

The thyroid is a butterfly-shaped gland located in the lower part of the neck. It produces hormones that regulate:

  • Metabolism
  • Body temperature
  • Heart rate
  • Energy levels
  • Growth and development

When diseases affect the thyroid gland, medications alone may not always be sufficient, and surgery becomes necessary.

When is Thyroidectomy Needed?

Patients may require thyroid surgery for several conditions.

Benign Thyroid Nodules

Large thyroid nodules that

  • Continue to grow
  • Cause cosmetic swelling
  • Cause pressure symptoms
  • Produce uncertain biopsy results

Thyroid Cancer

Surgery is the primary treatment for most thyroid cancers.

Examples include:

  • Papillary carcinoma
  • Follicular carcinoma
  • Medullary carcinoma
  • Selected cases of poorly differentiated thyroid cancer

Hyperthyroidism

Surgery may be recommended when

  • Medicines fail
  • Side effects occur
  • Large goitre is present
  • Graves' disease recurs
  • Patient prefers definitive treatment

Large Multinodular Goitre

Large thyroid enlargement may compress nearby structures causing

  • Difficulty swallowing
  • Breathing difficulty
  • Neck discomfort

Symptoms That May Need Evaluation

 Patients commonly notice

  • Neck swelling
  • Lump in front of the neck
  • Difficulty swallowing
  • Change in voice
  • Breathing difficulty
  • Persistent throat discomfort
  • Enlarged neck lymph nodes

Not every swelling is cancer, but every persistent thyroid swelling deserves evaluation.

Investigations Before Surgery

 Most patients undergo

  • Clinical examination
  • Thyroid function test (TSH, FT4)
  • Ultrasound neck
  • FNAC (Fine Needle Aspiration Cytology)
  • CT scan in selected patients
  • Laryngoscopy to assess vocal cord movement when indicated

Types of Thyroid Surgery

 Hemithyroidectomy

Only one half of the thyroid gland is removed.

Usually performed for

  • Solitary thyroid nodule
  • Follicular neoplasm
  • Selected indeterminate nodules

Total Thyroidectomy

Entire thyroid gland is removed.

Recommended for

  • Thyroid cancer
  • Bilateral multinodular goitre
  • Graves' disease
  • Bilateral thyroid disease

How is Thyroid Surgery Performed?

Modern thyroidectomy involves careful layer-by-layer dissection.

The surgeon identifies and preserves the important structures using magnification tools

  • Recurrent laryngeal nerve
  • External branch of superior laryngeal nerve
  • Parathyroid glands
  • Major blood vessels

Special surgical instruments help minimize bleeding and improve precision.

Benefits of Surgery

Depending on the disease, surgery can

  • Remove cancer
  • Relieve pressure symptoms
  • Cure hyperthyroidism
  • Improve cosmetic appearance
  • Prevent future complications

Possible Risks

Like all surgeries, thyroidectomy carries risks, although serious complications are uncommon.

Potential complications include

  • Bleeding
  • Infection
  • Temporary voice change
  • Permanent nerve injury (rare)
  • Temporary low calcium
  • Permanent hypocalcaemia (rare)
  • Scar formation
  • Need for lifelong thyroid hormone tablets after total thyroidectomy

Your surgeon will discuss these risks based on your individual condition.

Recovery After Surgery

 Most patients

  • Walk within a few hours
  • Resume oral diet the same day
  • Go home within 1–2 days
  • Return to office work in about one week
  • Resume strenuous activity after 2–4 weeks

Permanent voice problems are uncommon. Temporary hoarseness may occur due to nerve irritation or swelling.

Most patients experience mild to moderate discomfort that is well controlled with medications.

A scar is expected but usually fades over several months.

Only patients who undergo total thyroidectomy require lifelong thyroid hormone replacement.

Many thyroid cancers, particularly papillary thyroid carcinoma, have excellent long-term survival when treated appropriately.
